One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On 10/1/2019, employees of LHB Industries located at 8333 Fleischer Place, Berkeley, Missouri were exposed to hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  The following machines did not have guards:  1.) Logan Lathe horizontal lathes; Model# 6565 2.) Sears Craftsman 15 inch Drill Press, 12 speed ½ HP, Serial # 71344. 3.) Atlas Press Co. Drill Press, Model# 1060, Serial # 32221

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, were not constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section were exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top exceeded one-fourth inch:  On 10/1/2019, employees of LHB Industries located at 8333 Fleischer Place, Berkeley, Missouri, were exposed to struck-by hazards.  The tongue guard on a Dayton 6 Variable Speed Bench Grinder, Model Number 42672F, Serial Number 3207 was not adjusted to one-fourth inch.

29 CFR 1910.1200(f)(6)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with product identifier, words, pictures, symbols, or a combination thereof, which provide at least general information regarding the hazards of the chemicals:  On 10/1/2019, employees of LHB Industries located at 8333 Fleischer Place, Berkeley, Missouri, were exposed to chemical and fire hazards while working in the machine shop and laboratory areas.  A variety of chemicals in these areas were present without any form of label or markings as to the chemical identity.
