29 CFR 1910.217(b)(4)(i): Pedal mechanism(s) on mechanical power press(es) full revolution clutches were not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedals: (a) Facility, 201 Corliss Street, Pittsburgh, PA: On or about August 27, 2019 the foot pedal on the Bliss 19G 12 ton full revolution mechanical power press, machine #129 was not protected from unintended operation. An operator accidently cycled the machine when they stepped on the pedal when they had their hands in the die cleaning off debris. The employee sustained injuries to their right index and middle finger.

29 CFR 1910.217(e)(1)(i): The employer did not maintain a certification record of inspections which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the power press that was inspected: (a) Facility, 601 Corliss Street, Pittsburgh, PA: On or about October 1, 2019 the employer did not maintain any certification records of inspections of the full and part revolution mechanical power presses that included the date of inspection, signature of person performing the inspection, the serial number or other identifier of the press that was inspected. The employer does visual inspections of the equipment but does not maintain any written records.
