29 CFR 1910.217(b)(4)(i): Pedal mechanism(s) on mechanical power press(es) full revolution clutches were not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedals:    (a) Facility, 201 Corliss Street, Pittsburgh, PA:  On or about August 27, 2019 the foot pedal on the Bliss 19G 12 ton full revolution mechanical power press, machine #129 was not protected from unintended operation.  An operator accidently cycled the machine when they stepped on the pedal when they had their hands in the die cleaning off debris.  The employee sustained injuries to their right index and middle finger.
