29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Bradley Builders, Inc located at 3196 Saybrook Court, Dublin, OH 43017: On and preceding October 1, 2019, the employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower level were protected from falls. A laborer was assisting with the removal of sheathing and other materials on a roof that varied from 10-21 feet with the employees working at different levels. The employee was not utilizing proper fall protection which exposed them to falls up to 21 feet.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Bradley Builders, Inc located at 3196 Saybrook Court, Dublin, OH 43017: On and preceding October 1, 2019, the employer did not ensure that employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects utilized head protection. A laborer was assisting with the removal of sheathing and other materials on a roof that varied from 10-21 feet with the employees working at different levels. The employees were not utilizing proper head protection which exposed the them to struck by hazards to the head.

One employees was regularly engaged in work requiring fall protection.
