29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Andover Place Apartments, 10213 AltaVista Ave. - Employees in the process of roofing activities on a 6 in 12 slope (vertical to horizontal) roof, were exposed to a 29' 11" fall hazard, in that, fall protection equipment was not installed or utilized, observed on or about 10/01/2019. Fito Diaz Construction LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1342375, citation number 1, item number 1, and was affirmed as a final order on 2/20/2019, with respect to a workplace located at 18005 Altavista Ave., Tampa, Florida.

(a) Andover Place Apartments, 10213 AltaVista Ave. - Employees were exposed to a 29' 11" fall hazard, in that, the extension ladder did not extend 3 foot above the roof edge, observed on or about 10/01/2019. Fito Diaz Construction LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1342375, citation number 1, item number 2, and was affirmed as a final order on 2/20/2019, with respect to a workplace located at 18005 Altavista Ave., Tampa, Florida.
