On or about 09/25/2019, at the compressor room, employees were exposed to ammonia, in that, a determination was not made to assure that the exhaust ventilation system in the compressor room was in compliance with recognized and generally accepted good engineering practices after a change was made to the machine room by adding two compressors and a machine room tool vestibule which was separated from the machine room by a wall with an opening of 12 feet high by 22 feet long.
