29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On the east side of the construction site, employees engaged in painting activities used air pressure painting machine were not provided eye and face protection. The employees were exposed to eye injuries from paint spray.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On the east side of the construction site, employees performing painting activities were elevated in the basket of a JLG aerial lift without fall protection, exposing them to unprotected fall hazards of approximately 25 feet.
