This violation was most recently documented at a worksite located at or near 2220 County Road I, Crete, Nebraska 68333 on October 1, 2019. The process hazard analysis (PHA) was completed in January 2017. However, the PHA did not fully address qualitative evaluation of a range of the possible safety and health effects of failures of controls including, but not limited to; having an incomplete evaluation on the PHA Unit: Refrigeration System +34 Cooler Air Unit. There was an evaluation of the loss of containment if the unit was hit by a forklift or manlift; however, the consequences in regard to the possible injury did not detail what the actual injuries could be.
