29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: (a) At ironing machine numbers 1, 2, 3, 4, 5 and 6: On or about 10/2/2019, horizontal rotating shafts, located at both sides of the machines, approximately 37 inches above the floor and within approximately 18 inches of the employee, were not protected by stationary casings or enclosures, exposing employees to caught in hazards.

29 CFR 1910.1030(d)(1): Universal precautions were not observed to prevent contact with blood or other potentially infectious materials: (a) Ironing machine # 6: On or about 10/1/2019, an employee suffered a hand injury, causing blood to get on the machine and floor, and universal precautions were not used by a worker while cleaning the blood. (b) Receiving area: On or about 10/17/2019, employees in the receiving area of the laundry received soiled bedding from a nursing home. After receiving the soiled linens, employees proceeded to process the linens to be washed and dried, without using universal precautions, in that they used cotton gloves and no other personal protective equipment to process the linens.
