29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Travis AFB, Building 1205: Two employees were on a roof top installing clear panels without the use of fall protection. The employees were exposed to 13 and 22 foot falls.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: Note: Circumstances where retraining is required include, but are not limited to, situations where (1) Changes in the workplace render previous training obsolete; or (2) Changes in the types of fall protection systems or equipment to be used render previous training obsolete. Travis AFB, Building 1205: Retraining was not provided to employees when the fall protection system changed. Employees working on a metal roof were not provided with anchors or any type of fall protection. The employees were exposed to 13 foot and 22 foot falls.
