29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.    Employees were exposed to machine hazards associated with moving parts while conducting service and maintenance activities at the Sheboygan Magic Car Wash Full Service location when they had not received training as Authorized employees under the company Hazardous Energy Control Program.

29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.  Employees of the Sheboygan Magic Car Wash Full Service location have been conducting maintenance and service on equipment in a group Lock-out/Tag-Out scenario but have not been doing so under a procedure which affords each employee the same level of protection as a personal lockout or tagout device.

29 CFR 1910.242(b): Compressed air used for cleaning. Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.    On or about October 2, 2019, employees of Magic Car Wash were observed utilizing a compressed air line for cleaning purposes with a measured dead end pressure of over 100 p.s.i.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    On or about October 2, 2019 employees working in the Pump Room were exposed to potential electrical hazards:    a) when unused live electrical wires carrying over 50 volts were left hanging from the open end of a flexible electrical conduit with the exposed endings covered in electrical tape.    b) when a 480 volt electrical cabinet door was not shut and left the live wiring of the cabinet exposed.

29 CFR 1910.1200(f)(6): Workplace labeling. Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either: (i) The information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers; or, (ii) Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  On or about October 2, 2019, secondary containers throughout the facility did not include the name of substance inside of the container and a description of the hazards of that substance.
