29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points and rotating parts: a) 316 Voganville Road - The employer did not ensure that the Wafios CNC machine BM 51 L Serial # 2-7566-006 was properly guarded to protect operators from reaching in the compartment during operation. The limit switches designed to stop the machine if the lid is opened were disabled, on or about October 2, 2019. b) 316 Voganville Road - The employer did not ensure that the Wafios CNC machine BM 4 Serial # 2-7552-119 was properly guarded to protect operators from reaching in the compartment during operation. The limit switches designed to stop the machine if the lid is opened were disabled, on or about October 2, 2019. c) 316 Voganville Road - The employer did not ensure that the Wafios CNC machine BM 40 Serial # 2-7658-037 was properly guarded to protect operators from reaching in the compartment during operation. The limit switches designed to stop the machine if the lid is opened were disabled, on or about October 2, 2019. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) 316 Voganville Road - The employer did not ensure that they had the required energy control program to prevent employees from unexpected start-up or release of energy that could cause injury to employees who have performed maintenance on the Wafios CNC machines BM 51 L 2-7566-006, BM 4 2-7522-119 and BM 40 2-7658-037, on or about October 2, 2019. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
