29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Location: Southern Connecticut State University, 501 Crescent Street, New Haven, CT: The employer had not established and implemented written step-by-step hazardous energy control procedures in the machine specific, including alternative measures for the employees during servicing and maintenance of equipment, such as (but not limited to) the Ohio Medical air compressor unit at the off site location. On/or about 09-26-2019, the number 4 unit started up while one of employees was removing belts from the number 4 unit, causing severely broken finger and lacerations to the employee's right hand..
