29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section Location: Coin Sorting Room Employees were exposed to amputation hazards while performing maintenance on the Super Sorters where the cord and plug energy source is not unplugged, isolated, and the off button is being utilized to control the energy while employees are bypassing guards to complete the machine's necessary maintenance: Instances: a) Machine operators remove a guard from the drum and use compressed air to clean the machine by reaching their hand into the drum while the energy is not controlled by unplugging the machine. b) Approximately once per week, machine mechanics conduct maintenance on the Super Sorters by powering down the machine with the off button, removing the guard on the gear box, cleaning the components, powering up the machine, running a training process, and manually adjusting the belt in the gear box with their hands while the machine is running and the guard remains uninstalled
