29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   The employer is failing to protect employees from fall hazards while performing roofing activities.  This was most recently observed at 2604 S 87th Ave, Omaha, Nebraska on October 4, 2019 when employees were observed working on the roof of a residential structure without fall protection.  The roof eave height was over 7 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards while using ladders during roofing activities.  This was most recently observed at 2604 S 87th Ave, Omaha, Nebraska on October 4, 2019 when employees were using a ladder whose side rails did not extend at least 3 feet past the roof eave.
