29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards associated with residential roofing activities.  This was most recently observed at 2226 Du Pont St, Omaha, Nebraska on October 4, 2019 when 4 workers were installing roofing materials to a residential structure and they were not using fall protection.  The roof eave was over 9 feet high.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards while using ladders.  This was most recently observed at 2226 DuPont St, Omaha, Nebraska on October 4, 2019 when 4 workers were observed using a ladder to gain access to the roof of a residential structure and the ladder did not extend at least 3 feet past the upper landing surface.  The roof eave height was over 9 feet above the ground.
