On or about October 3, 2019, employees were exposed to trip and fall hazards in the following instances; (a): The bench area was not kept in orderly and clean condition. The walking space in the bench area was obstructed with boxes, equipment and chairs. (b): The warehouse passage ways are not kept in an orderly and clean condition. The walkways were blocked with boxes and various equipment.
