29 CFR 1910.1030(h)(1)(ii)(B): The medical record for an employee with occupational exposure did not include a copy of the employee's hepatitis B vaccination status, including the dates of all the hepatitis B vaccinations or any medical records relative to the employee's ability to receive vaccination as required by 29 CFR 1910.1030(f)(2): a) International Timber and Veneer, L.L.C., production areas, on or about October 3rd, 2019 and times prior thereto: Employees (first responders) had potential exposure to bloodborne pathogens intermittently throughout work operations including, but not limited to, responding to incidents involving blood, and/or cleaning up of blood and OPIM (other potentially infectious material). The hepatitis B vaccine and vaccination series was not made available to some employees with occupational exposure to blood and/or OPIM (other potentially infectious material). ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.1030(c)(2)(i)(A): The exposure determination did not include a list of all job classifications in which all employees in those job classifications had occupational exposure: a) International Timber and Veneer, L.L.C., production areas, on or about October 3rd, 2019 and times prior thereto: Employees (first responders) had potential exposure to bloodborne pathogens intermittently throughout work operations including, but not limited to, responding to incidents involving blood, and/or cleaning up of blood and OPIM (other potentially infectious material). The employers exposure determination list, within the written Exposure Control Plan, did not list first responders as employees that had occupational exposure onsite. Additionally, the written Exposure Control Plan listed departments in the exposure determination list that were not consider by the company as employees with occupational exposure to blood and/or OPIM (other potentially infectious material). ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
