MJM Solutions Midwest on or about October 3, 2019, at the address of 811 Hopley Ave., Bucyrus, Ohio:  The employer did not have a plan or program to identify job site hazards for employees to maintain a safe working environment.  Such a program would allow the employer to identify potential fall hazards during job site operations, decreasing the chance of employee injury.

MJM Solutions Midwest on or about October 3, 2019, at the address of 811 Hopley Ave., Bucyrus, Ohio:  A multi-ladder, used to access the roof of a commercial structure, was not utilized with a grasping device to assist employees in transition to/from the roof.  The ladder was not long enough to extend three feet above the landing service, exposing employees to fall hazards of approximately 20 feet.

29 CFR 1926.1053(b)(9): The area around the top and/or bottom of (a) ladder(s) was not kept clear:  a. MJM Solutions Midwest on or about October 3, 2019, at the address of 811 Hopley Ave., Bucyrus, Ohio:  The area at the top of the multi-ladder, used to access the roof of a commercial structure, was not kept clear in that the strap to secure the ladder to the roof obstructed the climbing rungs.  Employees were exposed to a 20 foot fall hazard when transitioning to/from the ladder.
