29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  The employer failed to ensure that the eyes of employees are protected from flying particles while working on a residential roof. This most recently occurred on or about October 03, 2019 on the jobsite located at 1757 84th Street, Omaha, Nebraska 68111.  Employee was observed using a hand-held pneumatic nail guns to perform roofing operations on a residential home without any form of eye protection exposing him to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards when conducting roofing operations on residential structures. This was most recently documented at 1757 84th Street, Omaha Nebraska on October 03, 2019 when employees were observed conducting roofing operations on a residential structure while working from heights greater than 12 and 15 feet without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof. This event was most recently documented at the jobsite located at 1757 84th Street Omaha, Nebraska 68111 on October 03, 2019. Employees were using 2 twenty-eight foot extension ladders to enter and exit the roof. These ladders did not have the side rails extended to at least three feet above the landing surface on the edge of the level of the roof.  The ground to eave heights were measured at approximately 12 feet and 15 feet.
