This event was most recently documented at the jobsite located at 1757 84th Street Omaha, Nebraska 68111 on October 03, 2019. Employees were using 2 twenty-eight foot extension ladders to enter and exit the roof. These ladders did not have the side rails extended to at least three feet above the landing surface on the edge of the level of the roof.  The ground to eave heights were measured at approximately 12 feet and 15 feet.
