29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): Jobsite: The 4 foot scaffold being used to access a door header that needed to be raised, was not fully planked. The work platform was a single plank measuring approximately 9 inch wide. On 10-4-19, An employee fell from the scaffold platform suffering serious injuries.
