29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds per employee attached, and was not installed under the supervision of a qualified person: a) Core Contractors, Inc at 5575 Tejon Street, Denver, CO 80221: On or before October 03, 2019, the employer did not ensure that anchorages used for attachment of personal fall arrest equipment was capable of supporting at least 5,000 pounds per employee attached, and was installed under the supervision of a qualified person. Two employees used the same anchorage point when installing snow fences on a commercial roof. This condition exposed employees to a fall hazard of 15-17 feet.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a) Core Contractors, Inc at 5575 Tejon Street, Denver, CO 80221: On or before October 03, 2019, the employer did not ensure that each employee exposed to fall hazards was trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used. Two employees were not trained on the correct use of personal fall arrest systems in that both used the same anchorage point when installing snow fences on a commercial roof. This condition exposed employees to a fall hazard of 15-17 feet.
