29 CFR 1926.452(w)(1): Scaffolds shall be braced by cross, horizontal, or diagonal braces, or combination thereof, to prevent racking or collapse of the scaffold and to secure vertical members together laterally so as to automatically square and align the vertical members. Scaffolds were not plumb, level, and squared:  ...In the alternative...  29 CFR 1926.451(c)(3): Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement:  (a) On or about October 4, 2019, at the worksite located at N92 W15800 Megal Dr., Menomonee Falls, WI, the employer did not ensure a scaffold system was plumb, thereby exposing workers to struck-by and fall hazards.  (b) On or about October 4, 2019, at the site address of N92 W 15800 Megal Dr., Menomonee Falls, WI, the employer did not ensure cross-braces were utilized at each location where installation points existed, thereby exposing workers to fall hazards and struck-by hazards.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  (a) On or about October 4, 2019, at the worksite located at N92 W15800 Megal Dr., Menomonee Falls, WI, the employer did not ensure a scaffold wagon had wheels that were secured with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement while used in a stationary manner, thereby exposing workers to struck-by hazards and fall hazards.

The wheels of a wagon scaffold were partially resting on wooded planks which were on soft soil, thereby exposing workers to fall hazards and struck-by hazards.
