The employees were exposed to struck-by and eye injuries.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about October 1, 2019, and at times prior there to, the employees working on a steep roof with unprotected sides and edges 6-feet or more above lower levels were not protected from falling at the east end of the newly constructed medical complex, employees were exposed to fall hazards of approximately 22-feet to the ground below.

On or about October 1, 2019, and at times prior thereto, employees performing roofing activity were exposed to fall hazards while ascending or descending from the roof due to an extension ladder that was not 3 feet above the landing.
