29 CFR 1926.760(d)(1): Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components did not conform to the criteria in 1926.502: a) Job site, 320 East North Avenue, Pittsburgh, PA: On or about October 1, 2019, during curtain wall installation activities, the employer did not ensure that appropriate fall protection systems/devices were in use while working near the perimeter of the top of the second story of the building in that the wire rope guardrail system in use was able to deflect down to a level below 39 inches.

29 CFR 1926.454(a): The employer did not each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Job site, 320 East North Avenue, Pittsburgh, PA: On or about October 1, 2019, during curtain wall installation activities, the employer did not ensure that employees were provided with appropriate scissor lift training in that an employee was determined to be within 1 to 2 feet of the lift as it was raised up/lowered down.
