29 CFR 1926.501(b)(2)(i): Each employee who was constructing leading edge work 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.   A) 10th Ave & Ocean Ave, Ocean City, NJ - Fourth Floor: Employees installing decking on the fourth floor were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems exposing them to a fall to the next lower level of approximately 10 feet.  B)10th Ave & Ocean Ave, Ocean City, NJ - Third: Employees installing decking and sheeting on the third floor were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems exposing them to a fall to the next lower level of approximately 18 feet.   Condition observed on or about 02OCT20.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels by personal fall arrest systems, covers, or guardrail systems erected around such holes:  A) 10th Ave & Ocean Ave, Ocean City, NJ - Second Floor: Employees working on the second floor were not protected from falling through open floor holes by guardrail systems, safety net systems, or personal fall arrest systems exposing them to a fall to the next lower level of approximately 13 feet.    Condition observed on or about 02OCT20.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not marked in a manner that readily identified them as defective, tagged with "Do Not Use" or similar language, or withdrawn from service until repaired.  A) 10th Ave & Ocean Ave, Ocean City, NJ - Second Floor: Employees were exposed to fall hazard(s) when using a Werner Ladder to access and egress the second floor that had split side rails.   Condition observed on or about 02OCT20.
