Adams St., West Chester, PA 19382 - The employer did not ensure that employees on a scaffold platform on the forks of a powered industrial truck approximately 14 feet above the lower level were protected from falls, thus exposing employees to fall from height hazards. This condition was noted on or about October 4, 2019.
