29 CFR 1910.178(g)(11): Precautions were not taken to prevent open flames, sparks, or electric arcs in battery charging areas.     On or about October 7, 2019, the employer did not ensure that precautions were taken to prevent electrical sparks or electrical arcs in the battering charging area.  The in-use battery charging cables for the Clark forklift had damaged insulation, exposing the internal copper wire on both the red and black cables. During the charging process of the forklift battery, flammable hydrogen gas is generated and could be ignited by electrical sparks or electrical arcs. This exposed employees to fire and explosion hazards.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.178(n)(4): The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing.    On or about October 7, 2019 the employer failed to ensure employees were sounding the horn at cross aisles and other locations. Employees were operating a Clark Forklift without a properly functioning horn.  The forklift is used several times per day and passes through aisles and doorways. Employees were exposed to struck-by hazards.     Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.    On or about October 7, 2019 ,the employer failed to a remove a Clark powered industrial truck in need of repair when employees were required to operate the forklift without proper functioning visual or audio devices. There was no proper functioning flashing, rotating or strobe lights, no back-up alarms when driving in reverse, and no functioning horn. Employees were exposed to struck-by injuries when operating the forklift.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
