(a) Building 140 - Mechanical Room 20 - On or about October 1, 2019 and at times prior thereto, employees were exposed to a fall of approximately 5' from the fixed ladder because a steam pipe was preventing employees from placing their foot securely on the ladder rung.

The fall distance is greater than 4 feet. (c) Building 485 - Mechanical Room - On or about October 9, 2019 and at times prior thereto, employees were exposed to falling through the ladderway access point while performing work such as: preventative maintenance and repair of the HVAC unit. The fall distance is greater than 4 feet.
