29 CFR 1910.23(d)(2): Employer did not install fixed ladders with a minimum perpendicular distance of 7 inches (18 cm) (for elevator pit ladders, a minimum perpendicular distance of 4.5 inches, 11 cm); from the centerline of the steps or rungs, or grab bars, or both, to the nearest permanent object in back of the ladder.    (a) Building 140 - Mechanical Room 20 - On or about October 1, 2019 and at times prior thereto, employees were exposed to a fall of approximately 5' from the fixed ladder because a steam pipe was preventing employees from placing their foot securely on the ladder rung.

29 CFR 1910.28(b)(3)(v)(A): The employer did not ensure each employee was protected from falling through a hatchway and chute floor hole by a hinged floor-hole cover that meets the criteria in � 1910.29 and a fixed guardrail system that leaves only one exposed side. When the hole is not in use, the employer must ensure the cover is closed or a removable guardrail system is provided on the exposed sides.    (a) Building 255 roof access at the east side center stairwell - On or about October 17, 2019 and at times prior thereto, employees were exposed to a fall up to 25' while working around the open roof hatch.

The fall distance is greater than 4 feet.      (c) Building 485 - Mechanical Room - On or about October 9, 2019 and at times prior thereto, employees were exposed to falling through the ladderway access point while performing work such as: preventative maintenance and repair of the HVAC unit. The fall distance is greater than 4 feet.
