On or about 10/02/2019, an employee used the welded support bracing as a ladder and climbed the tubular welded frame scaffold from the third tier to fourth tier of temporary scaffold.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Location - 52 Dunham Ridge Road, Beverly, MA. On or about 10/02/2019, project manager, foreman, and employees were working/walking on scaffolds and were not trained to recognize hazards such as fall hazards, incomplete decking, missing railings, four tier high scaffold not secured, employee climbing unsecured four tier high outside tubular post to gain access to upper level. An employee crossed over an open gap from temporary scaffold to main scaffold with no fall protection.
