29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a)  Veterans Affairs Tacoma Vet Center at 4619 Center Street Suite E Tacoma, WA --Asian Group Room;  A white surge protector (relocatable power tap) was plugged into a grey surge protector which was plugged into a white surge protector which powered lamps. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.   (b)  Veterans Affairs Tacoma Vet Center at 4619 Center Street Suite E Tacoma, WA-- Bobs Office;  A Midea refrigerator was plugged into a surge protector (relocatable power tap). The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.   (c)  Veterans Affairs Tacoma Vet Center at 4619 Center Street Suite E Tacoma, WA-- Bobs Office;  A Zeppoli electric kettle was plugged into an extension cord which was plugged into a surge protector (relocatable power tap). The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.   (d)  Veterans Affairs Tacoma Vet Center at 4619 Center Street Suite E Tacoma, WA --Break Room;  A Mr. Coffee Coffee pot was plugged into a surge protector (relocatable power tap). The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.    This exposes employees to potential electrical fire hazards.   The Vet Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1058248, citation number 1, item number 1 and was affirmed as a final order on July 28, 2015, with respect to a workplace located at Everett, Washington.
