29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency: a) Job site, 3290 Sawmill Run Blvd, Pittsburgh, PA 15227: On or about October 8, 2019, during commercial construction activities, the employer did not ensure proficiency in the topic of fall protection when using an aerial lift. Employee was approximately 7 feet above ground level.
