29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  A) 615 Wood Avenue, North Brunswick, New Jersey 08902  Employees conducting carpentry and framing work were exposed to trip and fall hazards due to construction materials and debris not kept in an orderly fashion. 29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  A) 615 Wood Avenue, North Brunswick New Jersey 08902  The employer did not ensure that the self-supporting fiberglass extension ladder was set with the side rails at least three feet above the landing surface exposing the employees to fall hazards of approximately 14 feet.    Condition noted on or about 10/07/2019.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure under another provision of paragraph 1926.501(b):  A)  615 Wood Avenue, North Brunswick NJ 08902   Employee(s) were exposed to fall hazards greater than 30 feet when installing framing on a two story home with no fall protection system in place.    Condition occurred on or about 10/07/2019.   V.Freitas Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1244002, citation number 1, item number 1 and was affirmed as a final order on 9/5/2019, with respect to a workplace located at 16 George St., Morristown, NJ on or about 6/27/2017.  V.Freitas Construction dba Freitas Framing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1007163, citation number 1, item number 1 and was affirmed as a final order on 1/13/2015, with respect to a workplace located at Chambers Bridge RD Nobility Crest at Brick, NJ on or about 11/05/2014.
