On and/or before October 1, 2019, the employer did not ensure that the portable Little Giant LT lightweight multi-use ladder system (Model: 22 LT; Part Number: 14322-01) was used as designed with the Tip & Glide wheels positioned to the top of the ladder when used in the straight ladder mode/configuration, thus exposing employees to a fall hazard. b. On and/or before October 1, 2019, the employer did not ensure that the portable Little Giant LT lightweight multi-use ladder system (Model: 22 LT; Part Number: 14322-01) was used as designed by fully extending the top outer ladder assembly first before the bottom section was extended in the straight ladder mode/configuration, thus exposing employees to a fall hazard.

29 CFR 1926.1053(b)(7): Ladders shall not be used on slippery surfaces unless secured or provided with slip-resistant feet to prevent accidental displacement. Slip-resistant feet shall not be used as a substitute for care in placing, lashing, or holding a ladder that is used upon slippery surfaces including, but not limited to, flat metal or concrete surfaces that are constructed so they cannot be prevented from becoming slippery. a. On or about October 1, 2019, the employer did not ensure that the portable Little Giant LT lightweight multi-use ladder system (Model: 22 LT; Part Number: 14322-01) was secured to prevent accidental displacement when used on a composite deck surface in the straight ladder mode/configuration, thus exposing an employee to a fall hazard. On or about October 1, 2019, an employee was performing modifications to a gutter system at a residential home when the Little Giant LT lightweight multi-use ladder slipped/kicked out at the base causing the employee to fall with the ladder to the composite deck below which resulted in injuries and hospitalization.
