29 CFR 1926.300(b)(1): Power operated tools that were designed to accommodate guards were not equipped with the guards when they were in use, to protect the operator and employees nearby from hazards such as ingoing nip point, rotating parts, flying chips, or sparks:       a)  The DeWalt grinder tool did not have a guard; employees were exposed to laceration hazard when using the grinder during repair of a roll-down garage door. 29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked between the front uprights and the guardrail supports:      a)  Working platforms on scaffolds were not fully planked; employees were standing on a single 20 inch wide scaffold plank exposed to 12 ft fall hazard. Employer was using two scaffolds (tubular welded frame) when repairing a roll-down garage door. 29 CFR 1926.451(f)(3):   Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift,  and after any occurrence which could affect a scaffolds structural integrity:      a)  On the tubular welded scaffold; a scaffold frame was cut through leaving a 1 inch gap. Employees were exposed to fall hazard and laceration hazard when using the scaffold during repair of a roll-down garage door.
