29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) For the employees operating the Model HI 29-120 horizontal lathe on or about October 9, 2019 and times prior thereto, the employer did not enforce the use of chuck guards to protect the operator from the rotating chucks.  b) For the employees operating the TOS Trechin Type SN 55B on or about October 9, 2019 and times prior thereto, the employer did not enforce the use of chuck guards to protect the operator from the rotating chucks.
