(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: Location - Mixing area On or about February 13, 2020, employees were exposed to a fall hazard greater than 4 feet to the ground, while working at the platform of the mixing machines.

29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized: Location - crimping machine area On or about February 13, 2020, an employee was exposed to noise levels exceeding the 8-hour time-weighted average (TWA) of 90dBA while working around crimping machine area. The employee was exposed to continuous noise level of 94.4dBA as an 8-hour Time-Weighted Average (TWA), which was equivalent to 185.3% of the permissible daily noise exposure during 438 minutes sampling period and zero exposure was assumed for the un-sampled time period. The employer did not implement engineering controls.
