29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level:    The employer is failing to ensure that each employee working on a scaffold more than 10 feet above a lower level was protected from falling. This was most recently documented on or about October 9, 2019 at a commercial construction site located at 7555 S 15th Street, Lincoln, NE 68512. The employer had not protected all outside edges of the platform with guard rails nor were employees utilizing personal fall arrest systems.

29 CFR 1926.451(g)(4)(ii): The top edge height of toprails on scaffolds were not installed between 38 and 45 inches above the platform surface:  The employer is failing to ensure that each employee working on a scaffold while performing plastering operations is protected by a guardrail installed between 38 and 45 inches above the platform surface.  This was most recently documented on or about October 9, 2019 at a commercial construction site located at 7555 S 15th Street, Lincoln, NE 68512. The employer had not protected all outside edges of the top platform with guardrails.

This was most recently documented on or about October 9, 2019 at a commercial construction site located at 7555 S 15th Street, Lincoln, NE 68512. The employer had not fully planked all working levels of the scaffold employees worked on to include the highest level which exceeded 10 feet.

29 CFR 1926.451(b)(3)(ii): The maximum distance from the face for plastering and lathing operations shall not exceed 18 inches (46 cm):      The employer is failing to ensure that each employee working on a scaffold were protected from falling while performing plastering operations on the exterior wall of a commercial structure. This was most recently documented on or about October 9, 2019 at a commercial construction site located at 7555 S 15th Street, Lincoln, NE 68512. The employer had constructed scaffold where an employee was performing plastering operations on the side of the structure that exceeded the 18 inches required between the structure and the scaffold.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    The employer is failing to ensure that each employee working on a scaffold were protected from falling when accessing the scaffold working levels. This was most recently documented on or about October 9, 2019 at a commercial construction site located at 7555 S 15th Street, Lincoln, NE 68512. From the ground to the first point of access on the scaffold, the two foot threshold had been exceeded. The employer had not provided a means of access such as ladders or walkways.
