Employees were working above other employees with tools and materials and the employer did not ensure that they were wearing protective helmets.

On or about October 10, 2019, employees were exposed to a fall of approximately 12 feet while performing siding activities on a residential home located at 2446 Nimisila, Clinton, Ohio 44216. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least three feet above the landing area.
