29 CFR 1926.451(b)(1): Each platform on all working levels of the scaffold was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  Location: a) Platform on 2nd tier of Scaffold (outside of the 2nd Floor), Front of Building.  a) On or about 10/08/2019 Employees used a scaffold, that was not fully planked, to perform façade/brick work on the exterior wall of the 2nd floor.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.   Location: a) Front yard ground level to 1st tier of scaffold (Outside of buildings main door)  a) On or about 10/08/2019, An employee climbed the scaffold frames and crossbraces to access the platform of the first tier on the scaffold.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Location: a) Outside of 2nd Floor, Front face of the building exterior (on wood placed on horizontal beams of balcony area)  a) On or about 10/08/2019, employees were stepping onto the permanent horizontal balcony beams that was covered with wood, on the front face of the building, to do exterior façade/brick work and to access the scaffold. No forms of fall protection or guardrail systems were used by the employees that stood on the balcony area.  b) On or about 10/08/2019, employees were on the platform to perform the exterior façade/brick work on the front face of the building. No forms of fall protection or guardrail systems were used by the employees that stood on this platform.
