29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) This violation occurred on or about June 1, 2019 and at times, prior thereto, specific written lockout/tagout procedures were not documented for the control of potentially hazardous energy when performing servicing and/or maintenance activities on the chopper machine and its components exposing employees to caught-in and struck by hazards. b) This violation occurred on or about October 3, 2019, specific written lockout/tagout procedures were not utilized for the control of potentially hazardous energy when performing unjamming of the chopper cyclone exposing employees to caught-in and struck by hazards.

29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: This violation occurred on or about October 3, 2019, the specific written lockout/tagout procedure for the Chopper machine failed to clearly identify all the energy sources which required an energy isolating device exposing employees to the hazards of unexpected energization of the equipment.

An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. This violation occurred on or about October 3, 2019, and at times prior thereto, employees were not required to shut off the main control panel for the chopper machine when unjamming the cyclones exposing employees to struck-by and caught-in hazards.
