29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On or about 09 October 2019, at a jobsite located at 407 Forest Ave. Oswego IL. 60543, employees of AC Roofing Services Corp. who were engaged in residential roofing operations were using an extension ladder that was not extended to at least three feet above the eave to allow for safe access to the roof. This exposed employees to a fall of approximately 11.5 feet feet onto concrete below. Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system On 09 October 2019, at a job site located at 407 Forest Ave, Oswego IL. 60543, employees of AC Roofing Services Corp. were exposed to a fall hazard in excess of 11 feet. Employees were observed from a public road removing roofing material on a residential roof and were not utilizing fall protection. Abatement documentation is required for this item in accordance with AC Roofing Services Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1247922, citation number 1, item number 1, and was affirmed as a final order on 10/03/2017, with respect to a workplace located at 12953 Kensington Drive Plainfield, IL 60585. AC Roofing Services Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1249676, citation number 1, item number 1 and was affirmed as a final order on 09/25/2017, with respect to a workplace located at 12952 Kensington Dr. Plainfield, IL 60585.
