Durez Corporation located at 13717 US Highway 68 South in Kenton, Ohio; On or about October 3, 2019 the employer did not ensure that that beams and piping systems located above the bulk bag pack machine located in building 51 were free of combustible resin dust. Employees were exposed to hazards associated with an explosion.

29 CFR 1910.22(a)(2): The employer did not ensure the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. When wet processes are used, drainage was not maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats were not provided:    a) Sumitomo Backlite North American, d.b.a. Durez Corporation located at 13717 US Highway 68 South in Kenton, Ohio; On or about October 11, 2019 the employer did not ensure that the floor at and around the small bag pack machine was free of combustible resin dust. Employees were exposed to trip and fall hazards.
