29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE. a) #6 tank void on the Tank Barge TB 2901 located at the main pier: On or about October 4, 2019 at 09:30 hrs. an employee was heating a piece of angle iron with a gas torch so it could be bent. Employee was wearing clothing of man-made fibers and not cotton or fire resistant clothing as required for welding/heating operations. After the oxygen hose burst it caused the shirt and jacket made of man-made fibers to melt on his lower stomach causing burns requiring skin grafts.

The incident was reported to OSHA on October 7, 2019 at 11:30 hours; 74 hours after the accident occurred.
