29 CFR 1926.451(g)(4)(iii): When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform: Worksite located at 449 Patton Ave., Springfield, Missouri- Employees were exposed to fall hazards in that the employees had failed to use the chain located on the end of the scissor lift that was to act as the midrail for the scissor lift.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Worksite located at 449 Patton Ave., Springfield, Missouri- Employee was observed working and walking along an unprotected edge and was not wearing any means of fall protection.
