29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):      On October 3, 2019, at the Walkers Food Products Facility, employees were exposed to amputation or other serious injuries from the bulk line filling machine, in that a guard was not designed and positioned to prevent the operator from having their hands enter the opening that dispenses product into tubs.

29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance:  On or about October 10, 2019, at Walker's Food Products, lockout/tagout procedures are performed by quality assurance personnel on a protein cutting machine and they have not been properly trained as authorized employees to perform lockout/tagout.

29 CFR 1910.147(c)(7): The employer shall provide training to ensure that the purpose and functions of the energy control program are understood by employees and that the knowledge and skills required for the safe application, useage, and removal of the energy controls are acquired by employees.    On or about October 10 2019, employees at Walkers Food Products were unable to demonstrate adequate knowledge and understanding of the purpose and function of the energy control program nor the knowledge and skills required for the safe application, usage, and removal of the energy controls.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    An examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified.  Specifically, reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources.
