29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: On or about October 7, 2019, employees were in use of a rough terrain forklift truck with a steel personnel lifting platform mounted to the forks. The employer did not have written approval from the rough terrain forklift truck manufacturer for this addition and this exposed the employees to a fall hazard.
