29 CFR 1910.178(n)(4): The driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing. On or about October 6, 2019, forklift operators did not slow down at the corner of dock doors 19 & 20 where there was limited view due to stored material exposing employees to struck by hazards.

29 CFR 1910.178(n)(8): Under all travel conditions the truck shall be operated at a speed that will permit it to be brought to a stop in a safe manner. On or about October 6, 2019, a forklift operator maneuvering a turn did not operate at a speed which permitted the Linde #117 (Model: H32CT, S/N: A113134H000T) propane forklift to come to a safe stop exposing employees to a struck by hazard.

On or about June 26, 2019 and thereafter, the Yale #116 (Model: GLC070VXNDSE088, S/N: C910V06538R) had a damaged tire on the front left side exposing employees to tip over hazards.
