29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection: a) Fort Lee Army Base: On about 10/11/2019, a ground fault circuit interrupter device used to supply power to a charger unit was used improperly in that the ground fault circuit interrupter device was plugged into the female end of a green two-wire (18 AWG) extension cord, as a result employees were possibly exposed to a shock hazard. No abatement certification or documentation required.

29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances were not of the three-wire type: a) Fort Lee Army Base: On about 10/11/2019, the female end of a green two-wire (18AWG) extension cord was plugged into the male end of a three-wire ground fault circuit interrupter pigtail to supply power to a charger unit. No abatement certification or documentation required.

29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: a) Fort Lee Army Base: On about 10/11/2019, the Lull Highlander Model 844C-42, Serial #99W21P22-2314, equipped with a standard carriage attachment and used with an attached personnel basket was modified without the manufacturer's written approval. Tie wire was used to prevent the fork support bar from sliding off the carriage frame, as a result employees were exposed to a potential hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
