29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)     Location:  a) Supported scaffold on first floor northeast side of the jobsite:     On or about 10/15/2019, employees were installing stuccos and painting on the third tier platform of a four-tier-high supported scaffold that was not fully planked or decked between front upright and the guardrail supports, and were exposed to falling approximately 18 feet to the lower level.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Scaffold frames were used as a means of access.    Location: a) Supported scaffold on first floor northeast side of the jobsite:    On or about 10/15/2019, employees climbed the scaffold frames and crossbraces as a mean of access between the third tier platform of a four-tier-high supported scaffold to the first floor level.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed.    Location:  a) On the front porch roof level of the jobsite:    On or about 10/15/2019, an employee used a folded A-frame ladder as a mean to install stucco and was exposed to falling. The ladder was not designed to be used while folded.     Location:  b) On the ground level of the jobsite:    On or about 10/15/2019, employees painted while standing on top of a plank, which was supported on one side by a step of an A-frame ladder and supported on the other side by the base of the handrail for the front entrance staircase. The ladder is not designed to be use for this purpose.
