29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 10/15/19, on the roofing jobsite of a house at 415 Norwood Avenue, Buffalo, NY: One employee engaged in roofing work on the steep roof at the front, south side of the house was exposed to a fall of approx. 18 ft. No fall protection system was being used. Pitch of the roof was approx. 14 in 12.   NO ABATEMENT DOCUMENTATION REQUIRED       David Ridgeway dba DNR Home Improvement was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1381918, citation number 2, item number 1 and was affirmed as a final order on 04/03/2019, with respect to a workplace located at 5599 Camp Road, Hamburg, NY 14075.    David Ridgeway dba DNR Home Improvement was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1238970, citation number 1, item number 2 and was affirmed as a final order on 10/17/2017, with respect to a workplace located at 4 Ashwood Court, Lancaster, NY 14086.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about 10/15/19, on the roofing jobsite of a house at 415 Norwood Avenue, Buffalo, NY: An extension ladder was set up on the front, right side of the house above the entrance way, and used by employee(s) to access the roof. The siderails of that extension ladder were only approx. 28 inches above the roof landing area.  NO ABATEMENT DOCUMENTATION REQUIRED      David Ridgeway dba DNR Home Improvement was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1290993, citation number 1, item number 2 and was affirmed as a final order on 03/05/2018, with respect to a workplace located at 11 Robert Drive, Lancaster, NY 14086.
